This year the annual Open Enrollment will be September 23 through October 31, 2014.
How to Enroll?
We are also introducing the new Fringe Benefits Management Company (FBMC). You can access the FBMC Enrollment site here: https://bmc.myfbmc.com.

Flexible Spending Account:
Employees will also have the opportunity to enroll in the Flexible Spending Account for 2015. Changes made during this period will be effective January 1, 2015. Remember, this is the only time you are able to make changes to your benefits during the plan year, unless a qualifying event occurs which permits a change (i.e. marriage, birth of child, loss of previous coverage, etc.).

What’s New This Year?
FSW is upgrading our offerings for both Dental and Vision insurance coverage. While Dental and Vision insurance will still be administered by Delta Dental and VSP respectively, if you enroll in Dental or Vision insurance for plan year 2015, you will have better coverage than ever before. Plan summaries and rates are provided below.
During this Open Enrollment, all Full-Time FSW employees will for the first time be able to elect 1X, 2X or 3X your annual salary in Supplemental Life/AD&D insurance without a medical evidence of insurability form!* This is a one-time opportunity to change your Supplemental Life Insurance coverage without a medical exam!
*Please remember the maximum amount of Supplemental Life Insurance is 3x your base salary (not to exceed $200,000).
This year, if you waive Medical coverage and enroll in the Dental/Vision (DV) Plan, you may choose to cover dependents for the first time, at your expense. Rates to cover dependents on the D/V Plan are provided below.

Would You Like To Reduce Your Out of Pocket Medical Cost, Reduce Taxes and Increase Your Take-Home Pay?
Enroll in the Flexible Spending Account (FSA). There is no cost to participate in the FSA, however, employees who elect the Debit Card option will be charged an annual fee of $12.00.
Medical Reimbursement Account: is an IRS approved plan that allows employees to set aside money on a tax free basis to pay for medical expenses. Some of the medical expenses that are eligible to be reimbursed include: Deductibles, Co-payments, Prescriptions, Mental Health Care, Orthodontist, Dental Expenses, Chiropractor Services and Vision Expenses including eye exams, glasses and contacts.
By paying for your out of pocket medical expenses with tax-free dollars, the average employee will save approximately 23% to 25%. Also, employees have access to the money they put in the FSA before it is taken out of your paycheck. For example, if you elect to put $600.00 into the FSA, you have access to the $600.00 on January 1st, before any deductions are withheld.
Dependent Care Account: Allows you to pay for daycare expenses for eligible children and adults. Qualified expenses include adult and child daycare centers, preschool and before/after school care. Employees who enroll in this plan are able to pay daycare expenses with Tax Free income resulting in savings of approximately 25% of the daycare costs.
If you currently have a Flex Spending Account or Dependent Care Account while employed with FSW, please enroll in the FSA for 2015 by completing the Online Open Enrollment via the FBMC website.
If you have never had a Flex Spending Account or Dependent Care Account while employed by the FSW, please complete the FSA Enrollment Form and return the completed document to Human Resources.
Employees interested in enrolling for Supplemental Life Insurance up to 1, 2 or 3X their salary may do so during Open Enrollment without Evidence of Insurability up to $200,000 worth of coverage. Exclusions may apply if you were previously declined or if you are on active leave. This is a voluntary, post-tax, employee paid benefit and the cost is $.277 per $1000 of coverage per month. To enroll in this benefit, complete the Mandatory Open Enrollment via the FBMC website.
Employees interested in applying for Short Term Disability Insurance may do so via Online Enrollment. This is a voluntary employee paid benefit and the cost is $.292 per $100 of monthly salary per month.
