Insurance Benefits

FSW makes various insurance plans available for full-time regular and full-time temporary employees. Insurance is effective the first day of the month following the date of hire. Premiums for employee paid benefits are payroll deducted one month in advance of coverage effective date.

Dependent children may be covered under FSW health insurance plans through the end of the calendar year in which they turn 30.

FSW pays the monthly premium for medical insurance for the employee. All plans are Health Care Reform Compliant.

If you choose to cover dependents under an FSW health insurance policy you will need to submit documentation to Human Resources to verify your relationship with those dependents. Please Note: A dental and vision (DV) plan is available to employees who have other primary medical insurance. Dependent coverage is available under the DV Plan at employee cost.

When declining medical insurance coverage, you will be required to sign a waiver of coverage in keeping with our commitment to compliance with the Affordable Care Act.

Dental Insurance


Dependent children may be covered under FSW dental insurance plans through the end of the calendar year in which they turn 26.

Two (2) Delta Dental plans are offered by FSW as a voluntary, employee paid benefit. Employees may choose to cover their dependent(s) under this policy within the first thirty (30) days of employment or during annual open enrollment. If you choose to cover dependents under an FSW health insurance policy you will need to submit documentation to Human Resources to verify your relationship with those dependents (PDF).

Vision Insurance

FSW offers a VSP vision plan. This is a voluntary, employee paid benefit offered to employees and their dependent(s) within the first thirty (30) days of employment or during annual open enrollment.

Please note that no vision card is provided for this plan and that providers verify coverage through VSP customer service (1-800-877-7195, Group #: 30035956).

Life Insurance


A $15,000 basic term life insurance and accidental death/dismemberment policy is provided by FSW with The Standard Insurance Company. Employees have the option of purchasing additional supplemental life insurance at one, two or three times their annual salary based on approval. Evidence of Insurability (EOI) may be required.

Long-Term and Short-Term Disability Insurance


Long-Term Disability Insurance

FSW pays 100% of the premium providing long-term disability insurance through Standard Insurance Company. This pays 66 2/3% of the employee's salary beginning on the 91st day after a continuous period of disability.

Employees may purchase short-term disability through the Standard Insurance Company during the first 30 days of employment.

Allstate Critical Illness Insurance (PDF)

With Allstate Critical Illness Insurance you will receive payment if you or your covered dependents experience events such as a heart attack, stroke, major organ transplant, cancer, or any other covered event. Allstate Accident Insurance (PDF)

Accident Insurance is a supplement to your health insurance in that it pays you directly for things like dislocations, fractures, hospital confinement, ambulance rides, burns, prosthetics, rehabilitation and much more. Flex Benefits Plan Enrollment Form (PDF)

Employees may participate in a Flexible Spending Account (FSA) and/or a Dependent Care Account to utilize pre-tax dollars for medical, dental, vision and dependent care expenses. The plans are administered by Custom Benefit Services and are voluntary, employee paid benefits.

Medical Reimbursement Account: is an IRS approved plan that allows employees to set aside money on a Tax Free basis to pay for Medical Expenses. Some of the Medical Expenses that are eligible to be reimbursed include: Deductibles, Co-payments, Prescriptions, Mental Health Care, Orthodontist, Dental Expenses, Chiropractor Services and Vision Expenses including eye exams, glasses and contacts.

By paying for your out of pocket Medical expenses with Tax-free dollars, the average employee will save approximately 23% to 25%. Also, employees have access to the money they put in the FSA before it is taken out of your paycheck. For example, if you elect to put $600.00 into the FSA, you have access to the $600.00 on January 1st, before any deductions are withheld.

Dependent Care Account: Allows you to pay for daycare expenses for eligible children and adults. Qualified expenses include adult and child daycare centers, preschool and before/after school care. Employees who enroll in this plan are able to pay daycare expenses with Tax Free income resulting in savings of approximately 25% of the daycare costs.

Cafeteria Plan (Section 125)


Under the Cafeteria Plan (Section 125) dependent health premiums, dental insurance premiums, and vision insurance premiums will be deducted prior to Federal Income or Social Security taxes being withheld. Certain guidelines must be followed.
