1. Please complete all of the demographic information required by you 2. Ensure that the waiver portion of the document is checked providing us permission to complete on your behalf 3. Have the document signed and dated appropriately. It needs to be a physical signature and not typed in. 4. Please send in a .pdf format 5. Provide the e-mail and/or fax number for the school’s office you are looking for admission into 6. E-mail all of this information to FSWCommunityStandards@fsw.edu
