Allstate Critical Illness Insurance (PDF)
With Allstate Critical Illness Insurance you will receive payment if you or your covered depednents experience events such as a heart attack, stroke, major organ transplant, cancer, or any other covered event. Premiums deducations for this benefit may be paid on a pre-tax basis.
Allstate Accident Insurance (PDF)
Accident Insurance is a supplement to your health insurance in that it pays you directly for things like dislocations, fractures, hospital confinement, ambulance rides, burns, prosthetics, rehabilitation and much more. Flexible Spending/Dependent Care Accounts

If you enroll in the High Deductible Health Plan with a Health Savings Account for your 2024 health insurance plan, you cannot enroll in a flexible spending account. You may still enroll in a dependent care account.
Flex Spending and Dependent Care accounts allow you to set aside a certain amount of each paycheck into an account on a pre-tax basis. The minimum annual election for the Flex Spending account is $250.00 while the maximum annual election amount for the Flex Spending account is $2,750.00. The maximum annual election for the Dependent Care account is $5,000.00.
Medical Reimbursement Account: MRA is an IRS approved plan that allows employees to set aside money on a pre-tax basis to pay for Medical Expenses. Some of the medical expenses that are eligible to be reimbursed include: deductibles, co-payments, prescriptions, mental health care, orthodontist, dental expenses, chiropractor services and vision expenses including eye exams, glasses and contacts.
By paying for your out of pocket medical expenses with pre-tax dollars, the average employee will save approximately 23% to 25%. Also, employees have access to the money they put in the FSA as of January 1, 2024. For example, if you elect to put $600.00 into the FSA, you have access to the $600.00 on January 1st, before any deductions are withheld.
Dependent Care Account: DCA allows you to pay for daycare expenses for eligible children and adults on a pre-tax basis. Qualified expenses include adult and child daycare centers, preschool and before/after school care. Employees who enroll in this plan are able to pay daycare expenses with pre-tax income resulting in savings of approximately 25% of the daycare costs.

Dental/Vision Plan (D/V Plan)
The "D/V Plan" is only available to FSW employees who choose not to enroll in a FSW health insurance plan.
Dental/Vision Plan participants will have the Delta Dental PPO and VSP vision plans.

2024 Vision Plans
Please note that no vision card is provided for this plan and that providers verify coverage through VSP customer service with your Social Security number.
2024 Supplemental Life Insurance Options
Employees who are currently enrolled in supplemental life insurance through FSW may increase their coverage amount without having to complete Evidence of Insurability during Open Enrollment as long as their total amount of coverage does not exceed $225,000. This is a voluntary, post-tax, employee paid benefit and the cost is $.277 per $1000 of coverage per month. Any new election, or increase in election amount will require employees to complete evidence of insurability (PDF). Sick Leave Pool
Employee may choose to enroll in the sick leave pool via the FBMC enrollment site during Open Enrollment each year.
