FAFSA Verification Process

Students who have been selected for a FAFSA process called verification must verify information reported on their application. In order to do so, students must complete requested tasks at https://fsw.studentforms.com. Please monitor your student email account for updates and any additional tasks to complete the verification process.

Change in Family Circumstance

Students who have experienced a loss of employment, a large reduction in income, or extraordinary medical expenses can complete a Family Contribution Appeal. The online appeal application can be found at https://fsw.studentforms.com and by selecting the “Manage Requests” icon. Appeals will be reviewed within two weeks of submission, please monitor your student email account for updates. 

Students who are seeking a financial aid appeal form, can locate the appeal forms at https://fsw.studentforms.com. The committee will review all appeals by the start of classes, if they are submitted by the priority deadlines.

Priority Submission Deadlines:

  • Fall: 08/06/2021
  • Spring: 12/16/2021
  • Summer: 4/30/2022

If, appeals are submitted after these deadlines, the appeal will be accepted through the end of add/drop, but there is no guarantee that the review will be completed in time to hold student’s classes.

Documents may be submitted via postal mail, fax, or in person to the Office of Student Financial Aid. Contact information for each campus location can be found here.
