Enrollment/Non-Enrollment Verification Request

Complete this form to request that FSW provides confirmation of your enrollment (or non-enrollment) to another institution of higher education, an employer, background-screening firm, or another third party.

Enrollment/Non-Enrollment Verification may be requested in three ways:

  1. By mail
  2. By fax (However, the faxed request must be accompanied by a release consent statement.)
  3. Place your request online via the National Student Clearinghouse.

NOTE:

  • FSW cannot provide verification of enrollment until the Add/Drop period for the term has passed. Add/Drop dates are listed in the Academic Calendar.
  • The FSW Office of the Registrar does not monitor a student's anticipated graduation date nor does the FSW Office of the Registrar require students to graduate within a specific time; therefore, an anticipated graduation date cannot be verified and is not included within FSW’s Enrollment Verification.

Petitions | Exceptions to Academic Policy

The petition process allows a student to formally request an exception to a policy or requirement. Generally, exceptions to policy are not made unless major extenuating circumstances are present, and supporting documentation exists. A standard form is completed and submitted along with all supporting documents. Documentation necessary for petitions can vary. In all cases, documentation must be official and must corroborate the justification given in an accompanying student statement; dates and other details must be consistent with the extenuating circumstances described in the student statement. Once a petition is reviewed, it becomes a part of the student’s permanent file.

Complete, signed petitions are reviewed monthly. Once a complete petition is received, it will be reviewed in line with the Petition Committee Review Schedule. Results (whether a petition is approved or denied) will be communicated to the student via her or his @Bucs email address.

Late Drop/Late Withdraw Petition Form

To file a petition requesting to Late Drop or Late Withdraw from a course, begin with this form. (Specific criteria apply.) NOTE: If you are a dual enrolled student, a different form will be required. Speak with your Student Success Advisor prior to submitting this petition.

Reduction of Fees Due to Continued Enrollment Petition Form

To file a petition requesting to waive the Multiple Attempt Fee associated with a third (or more) attempt of a course, begin with this form. (Specific criteria apply.)

Academic Second Chance Petition Form

To file a petition requesting to exclude from your student record the GPA associated with previous poor grades, begin with this form. (Specific criteria apply.)

Credit Review Request

The Credit Review process allows a student to formally request a re-evaluation of specific transfer credit. A Credit Review Request may be submitted by enrolled students who already have received an official evaluation of transfer credit.

The Credit Review process begins with an Academic Advisor. Complete, signed Credit Review Requests are reviewed monthly. Once a complete Credit Review Request is received, it will be reviewed in line with the Credit Review Committee Schedule. Results (whether a request is approved or denied) will be communicated to the student via her or his @Bucs email address.
