Authorization to Withhold Directory Information Form (a.k.a. "Confidential" Form)

Complete this form if you choose not to have any or all Directory Information released.

FSW may release to outside agencies or persons (upon request) Directory Information. Directory Information is information contained in a student’s education record which would not generally be considered harmful or an invasion of privacy if disclosed and may be disclosed without student consent. Information designated as Directory Information at FSW includes the following:

  • Student’s name.
  • Major.
  • Date(s) of enrollment.
  • Degree(s) and honors earned, with dates earned.
  • Participation in officially–recognized activities or sports.
  • Enrollment status (full-time/part-time).
  • Previous colleges attended.
  • Photographs.
  • Awards.

FSW will not release other information considered academic in nature without written permission of the student unless subpoenaed by a court or tribunal of competent jurisdiction.

Note: Once the Authorization to Withhold Directory Information form is filed, a student is considered “Confidential,” and all student services rendered must be carried out either in person or via videoconferencing software, as FSW staff members must uphold rigorous identification standards. Student services will not be provided over the phone.

  3. Place your request online via the National Student Clearinghouse.

FERPA Waiver Request

Complete this form to establish permission for FSW to release student record information to designated third parties (parents, spouse, etc.)

The Family Educational Rights and Privacy Act (FERPA) of 1974 prohibits an educational institution from releasing non-directory information about a student without the student’s consent. A student may waive this right for a third party by completing this form.

Note: This form may only be submitted by the student with photo identification either in person at the Office of the Registrar or live via videoconferencing software since strict, rigorous identification standards apply. Also note, this permission does not provide third parties with the ability to change, modify, or take action on a student’s record. Once this form is submitted, it is active until the student notifies the Office of the Registrar in writing to end permission.

Official Transcript Request

Complete this form to order your official FSW transcript.

Request to Inspect & Review Education Records

Complete this form to make a request to review your student record.

The Family Educational Rights and Privacy Act (FERPA) of 1974 is a federal law that protects the privacy of student education records. Under the provisions of FERPA, students are afforded the opportunity to inspect and review their records.

FSW complies with requests to inspect and review education records not more than 45 days after such a request is received and will make arrangements for access as well as notify the student of the date, time, and location for the review.

Petitions | Exceptions to Academic Policy

The petition process allows a student to formally request an exception to a policy or requirement. Generally, exceptions to policy are not made unless major extenuating circumstances are present, and supporting documentation exists. A standard form is completed and submitted along with all supporting documents. Documentation necessary for petitions can vary. In all cases, documentation must be official and must corroborate the justification given in an accompanying student statement; dates and other details must be consistent with the extenuating circumstances described in the student statement. Once a petition is reviewed, it becomes a part of the student’s permanent file.

Complete, signed petitions are reviewed monthly. Once a complete petition is received, it will be reviewed in line with the Petition Committee Review Schedule. Results (whether a petition is approved or denied) will be communicated to the student via her or his @Bucs email address.
